How do I locate a Personal Helpers and Mentors service provider?

The Department of Families, Housing, Community Services and Indigenous Affairs has completed the first two stages of a four-stage implementation process for the Personal Helpers and Mentors Program and services will continue to be rolled out across Australia over the next two years (between 2008 and 2010).

Service providers currently delivering the program are shown in the table below. The table also identifies the service provider's postcode boundaries (area serviced) and contact details.

How do I access the program?

People who live in a nominated postcode (as listed in the site links), can be referred to the Personal Helpers and Mentors Program through a range of other community services, such as disability employment service providers, clinical case managers, drug and alcohol support services, or housing support services.

People can also self-refer or be encouraged to access the program by their families or carers, or through a clinical pathway, such as after a period of hospitalisation. Potential participants will then undertake a functionally based assessment with the service provider to determine eligibility for the program.

If your postcode is currently not covered it may be included in future selection rounds

How were the sites selected?

The selection of the sites by FaHCSIA has been informed through joint consultation between FaHCSIA and the State and Territory Council of Australian Governments (COAG) Mental Health Groups.

The State and Territory COAG Mental Health Groups have endorsed sites in each state and territory, based on a range of factors, including:
The selection of sites has ensured there is limited duplication with programs funded by other jurisdictions, and a mix of metropolitan, rural, regional and remote locations.
